Athletic Registration Fees
Athletic Registration Fees offset the cost of transportation, equipment, supplies and other expenses associated with student athletics.
Any student at USC High School or Fort Couch Middle School who is participating in athletics will need to submit the 2017-2018 Athletic Registration Fee & Form linked below. Please take note of the multiple sports limits and the maximum fees that are listed on the form.
The form and fee should be submitted with the completed physical form to the Upper St. Clair High School Athletic Office.
NOTE: Sports physicals are required for students from Fort Couch Middle School and Upper St. Clair High School who plan to participate in interscholastic athletics.
In order to participate in school-sponsored athletic programs, the following documentation must be completed and submitted to the Athletics Office prior to participating in practices, scrimmages, and/or contests:
o Sports Physical: Sports physicals must be completed on or after June 1 of the upcoming school year and is valid for the entire school year. Sections 1-5 of the physical form should be completed by a parent/guardian and student. Section 6 must be completed by a physician. Multi-sport athletes will complete Section 7 prior to the start of each new sports season.
